Evaluates the patient's condition, coordinates and/or implements the plan of care, performs medical treatments, pain management and symptom control. Provides family and caregiver support. |

HOME HEALTH AIDE |

Provides direct personal care and assists patient/family with other activities of daily living under the supervision of the RN Case Manager. |
